Adam Dean is a new nurse manager on a med-surg unit. One nurse on the night shift is ill and cannot come to work. He contacts the area director, and the director floats a nurse from the mental health unit to his unit to assist. This nurse did work on a med-surg unit until 6 months ago. She and another nurse each have a patient receiving blood transfusions. The float nurse needs another unit of blood, and while the other nurse covers, she goes to the blood bank and signs out the unit of blood. Upon returning to the unit, her colleague is tied up in a procedure and says she will be out in about 20 minutes. After briefly conferring with her colleague, the float nurse hangs the blood on her patient. When her colleague completes her procedure, the two of them are in the process of checking the blood and discover that the blood obtained from the Blood Bank was for the other patient. The blood is stopped, the supervisor is notified as is the patient’s physician, and an incident report completed. As it turns out, the patient is a universal recipient. When Adam interviews each nurse individually, the nurse who administered the blood shows no remorse and tells Mr. Dean she cannot understand what t the issue is because nothing happened.
