ACUTE-GLOMERULONEPHRITIS Beau, who is 6-year-old, together with his mother presents to his pediatrician with the chief complain of two days facial, hand and fingers swelling. His mother first noticed swelling around her son’s eyes. The following day she noticed that her son’s urine appeared reddish -brown and also less urine output over the past seven days. The child has a history of streptococcal infection three weeks earlier. His medication is Dolan which his younger brother is taking whenever he has very high temperature to prevent seizures which Beau took one week ago for fever and sore throat that has been resolved. On examination, he is febrile with a HR of 89 beats per minute and a blood pressure of 140/95 mmhg. Beau has a periorbital edema. His fundoscopic examination is normal. His chest is clear to auscultation. His heart rhythm is regular, with a nondisplaced point of maximal pulse and he has no abdominal masses or bruits. The patient also has edema of his feet, hands and face. A dipstick urinalysis in the clinic shows a specific gravity 0f 1.025 with +2 blood and 2+ protein. A fresh spun urine specimen Is positive for red blood cell casts. A serologic test has a traced of an increased BUN and Creatinine The physician diagnoses acute glomerulonephritis. Beau is placed on a moderate sodium restriction. He is also prescribed oral penicillin G at a dosage of 125 mg 3 times a day for 7 to 10 days to eradicate the nephritogenic strain.
