The nurse noticed it and immediately stopped the Dopamine drip and removed the IV access. IV infiltrate therapy was implemented, but the harm was already done, there was little response to treatment and baby ended up losing the first phalange of three fingers. After, the root cause analysis, nurses were educated in detection, monitoring, documentation of IV infiltrates, and a strict CQI was reinforced; IV sites must be assessed every 30 to 60 minutes and documented, IV pumps’ alarms must be audible with alarms every one to two hours depending on the flow rate. This CQI is ongoing and everyone in the unit is held accountable for immediately responding to alarms. Since, then there has been quick detection of IV infiltrates and immediate intervention.
