Nurses have the responsibility to make patient care decisions on a daily basis, and it is necessary that they incorporate evidence-based research in their clinical nursing practice (Houser, 2018). One clinical situation where I was concerned was at the skilled nursing facility. The incident included an older woman who was recently admitted to the facility and in one of the mornings when we got to the facility she fell from the bed. The fall happened about an hour prior to us getting to the facility, and when I and another student approached the woman the nurse asked us to perform a neurological assessment on the patient. Being the first time in a clinical setting, we didn’t feel comfortable performing an initial neurological assessment on a patient who just fell and had a significant injury to her head. The decision we made, was to ask our clinical instructor to be with us and help with the assessment. Our clinical instructor recognized our concerns and agreed that the given assignment to us, was not safe for the patient. She assessed the patient and suggested to the nurse to contact the doctor and perform a detailed exam of the injury. The skill to apply evidence-based research in clinical practice can be mastered through repeated practice, application of the nursing curriculum, and feedback from instructors (Houser, 2018). In this situation, the source of our decision was the instructor’s advice which always tells us to never do something that is not safe for the patient and always ask questions or for help if needed. Due to biological changes, older adults have a higher risk of falling. “About half of nursing home residents suffer a fall at least once a year” (Schoberer et al., 2022). Besides physical consequences, falls also have psychological and social consequences. One of the psychological consequences is fear of falling which can lead to a reduction in both physical and social activities (Schoberer et al., 2022). It is important to be aware of these consequences and make sure to implement all necessary practices and follow the guidelines to prevent falls also in case it happened It is crucial to take proper actions to reduce the further complications of the fall.
