History of Present Illness: A 53-year-old woman presents with complaints of gnawing and constant 6/10 abdominal pain that started the day prior. She states she is afraid to eat anything for fear the pain will worsen. She reports an episode similar to this occurring 5 years ago after returning home from overseas travel. She recalls being told by a physician that she “probably had food poisoning”, being given an antibiotic, and feeling better 2-3 days later. She says this has not happened since. Her last meal was a breakfast of rye toast and yogurt with blackberries yesterday. She reports feeling feverish, sweaty and nauseated when the pain first started. She denies vomiting but does admit to a few bouts of dry heaves. She says the pain is “all over”. She admits to feeling bloated and having multiple bowel movements- last one prior to visit. Her stools have gone from soft to loose thin ribbons. Review of Systems: Positive for subjective fever with chills, diaphoresis, difficulty sleeping due to abdominal pain, dry mouth, nausea and reduced appetite. Abdomen: distended abdomen, well healed surgical scars in the mid-epigastric region, RUQ and suprapubic region, hyperactive BS x 4, diffusely tender mid to LLQ with rebound tenderness, no CVAT Question a.
