Original Question

You are a 3rd year student nurse on your 1st semester, final year placement on the Cardiac ward. It is now 1030 on your early shift and you are allocated 3 patients. One of your patients is Mr Turner who was admitted with an acute Anterior MI 3 days ago, for which he had a femoral angiogram, angioplasty and stent inserted to his Left Anterior descending artery on Day 1 of his admission. He has a PMH of AF, T2 DM, CVA and OA. His last set of observations at 1000 were RR 16, SpO2 98% on 2L via nasal specs, HR 75 and regular, BP 110/75 and temp 35.8, his groin site is soft and dry. It is now 1030 on your early shift and Mr Turner has just rung his bell. When you enter his room, he tells you that he fell over coming back from the toilet. Mr Turner is now sitting on the edge of his bed and you notice that he has a slight abrasion on the right side of his forehead and appears quite SOB. On taking his observations post fall his vital signs are RR 24, SpO2 95% on Room air, HR 125 and irregular, BP 90/60 and temp 36. He is complaining of pain in his chest and Right arm, which he is holding and appears to be swollen (not previously swollen). How can Mr. Turner might have APO, DVT & PE? Hypotension?
