1)- Please respond to the discussion post below in a paragraph or so. in a favor or adding something to it .. Provide references if any.. I will be attending clinical at CHI Primary Clinic in Omaha, Nebraska. This clinic provides family care to CHI employees and their families. There are no age restrictions in this clinic as the APRNs and MDs are trained to treat all ages. My clinical schedule will vary based on my preceptor’s schedule, but I am hoping to work at least three days a week. Hours range from 8am to 4 or 5pm. Unfortunately, I cannot start clinical days until May 10th but I am excited to begin and get as many hours as possible in. I am fortunate in not having to share my preceptor. She only takes one student at a time. 2. My goals for practicum are broad. This is my first experience outside of the acture care setting, so I am anxious to see a whole new form of patient care. My goals for the first day are to observe the time management of my preceptor and find a way to utilize my skills in forward thinking about care plans and addressing patient needs. I would love to be able to discuss my frame of thinking and assessing where it compares to my preceptors. By the end of the session, I hope to be comfortable assessing patients and utilizing clinical guidelines to find possible solutions. Using the Clinical Skills Checklist, I can review this with my preceptor to assure that we are on the same page about what skills I should be attempting and working on competency. This checklist is helpful in guiding skill competency. 3. Compared to undergraduate rotation, I expect this clinical to have higher expectations and more opportunities for growth. Because we are all licensed professionals, I hope our preceptors have more confidence in our abilities to help within the APRN role. When facing challenges in practicum, I would best describe my personality as humble, determined, open-minded and coachable. I want to be a blank canvas and open to all criticism when learning new skills and protocols. Two factors that my inhibit my completion of clinical requirements is limited patient availability in two ways. First there may not be enough patients seen within the eight weeks to get my hours. And some patients may not want a student in the room with them. To overcome this, I will adjust my schedule with either more or less days needed to reach my clinical requirement of 125 hours. I will reach out to my preceptor and ask if I can work extra days to meet my requirement. Also, if patient’s do not feel comfortable with students, I will find a way to involve myself into their care. Respecting their wishes and removing myself from the room, I can still review their chart with the NP and aid with care plan recommendations or researching best practices. 4. Evidence-based guidelines are a requirement when providing patient care. I plan on using Uptodate.com as a resource in identifying the best practices for patient care. Also, the AAFP clinical practice guidelines are a useful tool when addressing common chief complaints seen in the family care setting. Epocrates is another useful tool to assess diagnostics and disease pathology. The CDC has clinical practice guidelines as well that help providers identify the first line of treatment for diagnoses. Lastly, I utilize Pubmed to find supporting research and recent evidence-based practice techniques.
