Original Question

Kathie is a 67-year-old woman and lives alone. She married once but has no children. She has been increasingly depressed since her father’s death a year ago. She had lived with him for 18 years and has difficulty adjusting to life without him. About 6 months ago, Kathie admitted herself to a hospital when she became increasingly suicidal. She wanted to take her life by drug overdose but called 911 right before carrying out the plan. She was then hospitalized for a week. Her family history reveals that her mother had bipolar I disorder and now lives in a nursing home. Her younger sister was diagnosed with bulimia and had been treated for the condition till her late thirties. Kathie reports that her sister is now completely recovered and no longer needs treatment. During the intake assessment, Kathie is tearful and talks about how much she misses her father. She reports feeling sad, loss of interest in daily activities, difficulty sleeping, poor concentration, and feeling of hopelessness. These symptoms have been present almost daily for the past two months. Kathie thinks her medication Sertraline (Zoloft) 200mg PO QD that she has been on for the past 6 months seems not working for her. Kathie also has been suffering from insomnia for about a year and used to take a variety of sleeping pills. She is now taking Zolpidem (Ambien) 10mg PO at night for sleep. She usually gets in bed around 11pm but cannot fall asleep till 2am. She usually sleeps 4-5 hours at most. There is no apparent abnormality on mental status and physical exams. Kathie is 5-feet and 3 inches tall and weighs 142 pounds. Questions: 1) Using the DSM V diagnostic criteria, what is/are the diagnostic possibilities appropriate for this case study?
