Background: At 9:30 PM Judy, a 19 year old, was admitted to her local hospital and placed on a medical unit with complaints of stomach pain which she had experienced for the past 2 months. Judy’s mother told the nurse that her daughter had been depressed for the past six months but refused to see a psychiatrist. Judy’s mother left the room when the nurse started the admission interview. Judy stated that her boyfriend of a year broke up with her six months ago for another woman. Judy told the nurse, “I’m not pretty or fun-loving. I feel best when I drink a bottle of wine at night which I hide from my parents because they have tried to be very supportive of me.” Judy further stated, “I have no energy and my grades are falling. I’m just worthless.” The nurse asked Judy if she had thoughts of harming herself. Judy stated, “No I won’t harm myself.” Tried that once and I only ended up in ICU.” Judy said she just wanted to go to bed. The nurse made her comfortable and reassured Judy that she was available to talk and to just put her light on if she needed anything. The nurse then talked with Judy’s mother who collaborated that Judy’s boyfriend broke up with her 6 months ago and that is when Judy became depressed. She stated that Judy continued to attend her college classes but that her grades were dropping and she was withdrawing from her girlfriends and family. Judy’s mother further stated that Judy told her after the breakup that she was unlovable. Judy’s mother also stated that Judy made a serious suicide attempt when she was 15 years old by ingesting a bottle of Tylenol. She was transported via paramedics to the hospital and was placed in ICU for three days. She was admitted to an adolescent mental health unit for another three day stay. She was discharged when she reassured the psychiatrist that she was no longer suicidal. She was given a prescription for Citalopram (Celexa) which she did not take. Judy’s mother stated that Judy never talked about the attempt and she did not know what precipitated that attempt. There were no more attempts. Judy’s mother also stated that depression was in her husband’s family and that her mother-in-law committed suicide 10 years ago. Judy’s mother stated that the family is so concerned and they love Judy dearly. She stated that all of Judy’s friends were also worried and were at a loss as how to help their friend. The nurse then returned to the room and administered the Suicide Assessment Five-step Evaluation and Triage (SAFE-T). The results showed a high risk for self-harm, suicide. The nurse called the doctor for orders and reported the results of the SAFE-T. She requested that the doctor order a sitter. The doctor agreed and he started Judy on Bupropion (Wellbutrin). None of Judy’s belongings were inspected and Judy was allowed to keep all of her things including her robe with a tie, perfume bottle, nail file, Kleenex, keys and her wallet. The nurse told the sitter that Judy was suicidal and that she needed constant supervision. The nurse told her to put the light on when she needed a break and the nurse would arrange to have her relieved. At 8:00 PM the sitter put on the light and asked to go on her break. The nurse sent in a nursing assistant to relieve her after giving her a report on the patient. At 8:45 PM the sitter returned and the nursing assistant went about her duties of caring for other patients. At 11:00 PM the evening shift nurse gave report to the night shift nurse and stated that a relief sitter would arrive at 11:30 PM to relive the evening sitter. The night nurse made rounds and checked on Judy at 11:45 and the night sitter was with the patient. The night nurse was just about to give her report when an Emergency CODE was called on another patient on the unit. The nurse returned at 1:00 PM to find the sitter asleep and Judy was not in her bed. The nurse checked the bathroom and found that Judy hanged herself with her bathrobe tie. The nurse cut the patient down and told the sitter, who was now awake to call an Emergency Code. The nurse started CPR and the emergency team arrived.
