Reflect on the formal and informal feedback you received from your Preceptor ============================================================================================= My Practicum Experience Plan goals were the proper diagnoses consistent with the DSM 5 for the clients and to provide them with the appropriate treatment plan for the clients’ symptoms. My next goal was to be able to recognize the signs and symptoms of mental disorders. The final goal was to select an appropriate psychotherapeutic medication and assess the patient’s response to their prescribed medications. The challenging patient met during my practicum experience was a 16-year-old female diagnosed with Oppositional Defiant Disorder. She came in with her mother, and the mother insisted her daughter had ADHD and Bipolar disorder. My preceptor tried to explain to the mother what kind of symptoms her daughter was presenting and why he recommended the psychotherapy (Haine-Schlagel, & Walsh, 2015). The second, an older woman in her late 70s who suffered from MDD and Bipolar I, came in for the session feeling her mood change from sad to happy each time she comes in the appointment. It is hard to find proper medication to get them to a balanced mood. In another case, a female came in suffering from a borderline personality disorder with depression type. The client was not satisfied with her current medications even though the medicines have been changed several times. I have learned some clients’ mothers can interfere with their child’s treatment. The latest challenging parent was one who exhibited ADHD; rapid speech unable to stay on one topic. He decided only to take half of the dosage ordered but knew he needed the whole medicine. The preceptor does not have much time to see patients. Keeping on the topic was hard, and I learned a preceptor must set limits and attempt to keep on topic. The resources available where my preceptor was knowledgeable. He shares his idea with me. He recommended using DSM-5 and Stahl books. The evidence-based practice that I referenced is Stahl’s medication book and psychotherapy. It has been researched that psychotherapy alongside medication has improved knowledge. Clinical studies show that psychotherapy and medication can have better benefits for depressed clients than pharmacologic treatment alone (Dunlop, 2016). I can improve my skills and knowledge by taking more involved diagnoses clients instead of waiting on my preceptor to tell me what it is and allowing me to come up with the best treatment option for the client, or at least trying input which is the best way to learn to be prepared someday to be better PMHNP. Managing patient flow and volume can be difficult since setting patients’ appointments. The preceptor was scheduled to see twenties patients a day. Setting limits with patients is essential. My growing skillset can be used as a social change in my community, being an advocate for mental health (Badanapurkar et al., 2021). I want to discuss the stigma of mental health. I have learned a lot during this clinical rotation and am grateful for the preceptor I had. I feel this practicum has prepared me a little close to being a better PMHNP.
