He has right-sided hemiplegia with little voluntary movement on that side. In the last 24 hours, J.L. has experienced frequent liquid stools. Upon entering the room at the beginning of the shift, the nurse notices a strong fecal odor. J.L. has had decreased appetite in the last week, eating only small amounts of each meal. He is repositioned in bed by the CNAs every two hours and is assisted to his chair by Hoyer lift for two-hour increments 2-3 times a day as tolerated. What data from the history is RELEVANT and must be interpreted as clinically significant by the nurse? List at least three pieces of data.
