He said he felt ‘sick’ when I took his vital signs at 0600. Probably he just needs some orange juice.” The day nurse from an agency is not familiar with the residents. The only thing she knows from the shift report is that Mr. Stewart is 80 years old, has diabetes, and had vital signs within normal limits at 0600. 1. Nurse A, realizing that the regular staff probably knew the residents better, she followed the night nurse’s advice and gave Mr. Stewart some orange juice right away, before making rounds on her other residents. After making rounds she returned to see if he was feeling better. 2. Nurse B, realizing that she was not an expert on diabetes, immediately called Mr. Stewart’s physician and relayed the night nurse’s report so that she could get orders about how to proceed. 3. Nurse C went to Mr. Stewart’s room immediately after report, took his blood glucose level and vital signs, listened to his heart and lungs, and asked him to describe how he was feeling. Unable to recall the symptoms of low blood glucose, she reviewed and administered the appropriate treatment based on the hypoglycemic protocol. Her plan was to call the physician if Mr. Stewart’s condition did not improve within the hour. 4. Nurse D gave Mr. Stewart the orange juice immediately.
