The Lean model focuses on “eliminating waste from the production system by designing the most efficient and effective system”. There are many aspects of this in clinical settings, such as money, resources, or even time. The Lean model uses the Plan-Do-Study-Act (PDSA) Improvement Cycle. This is the last step of the improvement methods, which is to test and implement. Some specific issues I’ve noticed as a nursing student in the hospital setting is the nursing shortage and waste of resources such as money and inventory. “According to lean principles, any time patients or employees are required to stand by, waste happens”. A lot of time gets wasted because nurses have a higher workload and patients have more demands, therefore patient care becomes affected. On a busy day, a patient I was assigned to was in a lot of pain from surgery and needed her pain medicine to perform her ADL’s, which include showering and grooming, and a check-up with her OT to assess her performance. All of this happened a couple hours past the appointed time because the nurse was busy with other patients. There was also a time when a patient had to wait an hour past her scheduled time for her antibiotic. I’m not exactly sure how to improve or change the issue of this shortage, but I do know that using the principles of the “lean” idea would help to “identify areas of waste and eliminate anything that does not add value for patients”. Additionally, there are times when patient’s receive unnecessary treatments that increase time, effort, and resources. For example, my patient that I mentioned above was admitted partly due to polypharmacy or overmedication, which caused her to collapse. There is also the issue of keeping too much equipment at the bedside.
