Patient ambulated to room, oriented to room, call bell, procedural timeline of care, and given hospital gown to change into. 0745 Patient in bed, placed on fetal monitors. Vital signs taken at this time. 0755 18-gauge IV placed in LFA (Left Forearm) with NS at 125 mL/hr, and 18-gauge INT placed in right hand. Blood drawn and sent to lab. Provider at the bedside discussing next steps with the patient and family, including Pitocin induction and a magnesium infusion to prevent seizure activity. Provider performs sterile vaginal exam, states patient is 1cm dilated, 60% effaced, and at -1 station. 0840 At bedside to begin oxytocin labor induction and magnesium sulfate. Oxytocin infusion is started at 1 mU/min. Magnesium sulfate bolus is started at 4 g. Seizure precautions in place. Nurse remains at the bedside to monitor. 0850 Lungs clear to auscultation bilaterally, DTR +3, no clonus, and no new patient complaints. 0900 Lungs clear to auscultation bilaterally, DTR +2, no clonus. Patient voided 270 mL clear yellow urine in bedpan. Patient states she is feeling tired and nauseas, given ice pack for neck, and encouraged to take deep, slow breaths. Patient is positioned right lateral recumbent. Magnesium bolus completed. Continuous infusion of 2 g/hr started. 0930 Oxytocin infusion increased to 3 mU/min. Patient states nausea has resolved and has no needs at this time. Patient remains right lateral recumbent. 0935 Fetal heart monitoring indicates a category two tracing with minimal variability, baseline of 120, and episodic variables. Contractions every 5 to 8 minutes, mild on palpation.
