The patient became lethargic and confused, so the nurse called the physician. The physician ordered the lidocaine to be discontinued and the rate of the nitroglycerin to be increased. The patient became stuporous, still mumbling about chest pain. At this point, the nurse realized what the problem was. The night nurse had reversed the infusion bags on the pumps, subsequently, the staff had been relying on the labels on the IV pumps, and had not checked the labels on the bags.
