A 62 year-old male presents to the office as a new patient. He has not seen a health care provider in 10 years. The only complaint is progressive fatigue and generalized weakness. He denies any known past medical problems and takes no medications or OTC products. On exam, he has a pale appearance. HEENT exam is remarkable for a thick, beefy and quite erythematous tongue. There is a loss of proprioceptive sense on neurologic testing and the remainder of the exam in unremarkable. Initial labs reveal the following- CBC with WBC count 6.5/ cubic mm with normal differential, hemoglobin 7.2 g/dL with elevated MCV 102 (normal 80-86). Electrolytes and LFTs are normal.
