Original Question
There are many frameworks that can be used to obtain a comprehensive pain assessment. Some are multi-dimensional pain assessment/history tools such as the Initial Pain Assessment Tool and some are population specific (e.g., WOMAC for arthritic pain) tools. Most of these tools address some, but not all the important dimensions of pain. To perform a comprehensive assessment with these multi-dimensional tools, some additional questions may need to be added. Therefore, in lieu of these tools, many health care professionals use acronyms (e.g., OPQRSTU, OldCart) as a framework to perform a comprehensive pain assessment. The purpose of these various types of frameworks is to serve as a guide to ensure important dimensions of pain are assessed so that you can determine the type of pain the patient is having, select or recommend appropriate interventions and evaluate the effectiveness of your interventions. Try a couple of the frameworks or multi-dimensional pain assessment/history tools in clinical practice. If you aren’t seeing patients clinically this semester, use the framework with friends, or family members experiencing pain. Apply the framework using a case scenario to demonstrate how the framework: 1) provides information about pain characteristics (i.e., provide assessment findings for each component); 2) assists in the identification of the type of pain; 3) assists in determining the sensory-discriminative, affective-motivational, and evaluative-cognitive dimensions of pain; 4) is useful in evaluating the effectiveness of interventions; and 5) assists in communicating the patient’s pain to other health care professionals, thus allowing you to advocate for a change in the pain management plan. In your posting, also discuss 6) any shortcomings of the framework and how you will modify this assessment process to obtain a comprehensive pain assessment. To demonstrate how to use a comprehensive pain assessment framework in various populations, we need to address a comprehensive pain assessment in all of adult non-demented patient who can’t self-report (Osteoarthritis).
