You are a member of Bethune Hospital’s ethics committee. As a member you meet with the team to discuss ethical concerns that occur in the hospital. The family of Marjorie has requested that the Ethics Committee meet to review their mother’s case and offer a recommendation to the family and the healthcare team. All members of the Ethics Committee are present along with Marjorie’s family and the ICU attending physician overseeing her current care. The family has requested a staff nurse, Felicia, present the case. Marjorie was a spry and independent 87-year-old woman recently diagnosed with pancreatic cancer. At the time of her diagnosis, she was very practical and strong minded. She was offered a Whipple procedure as a possible cure. As Marjorie understood the situation, she would either die a lingering death from cancer, be cured by the surgery, or die “under the knife.” Informed consent was obtained by the surgeon and Marjorie appointed a financial power of attorney. Marjorie did not die on the table or from the surgery, but she has lingered for seven months in and out of the ICU as a result of successive complications. She is conscious but unable to communicate. As each complication arises it is treated individually. All of Marjorie’s complications are considered “potentially treatable” since her cancer is gone. Most of the nursing staff are disturbed by Marjorie’s long suffering, as are her friends. However, some of her family doesn’t agree. Most feel that Marjorie wouldn’t want to continue with aggressive treatment stating how active Marjorie was prior to surgery noting her fortitude and desire to live life to the fullest, but Marjorie’s sister disagrees and feels strongly that they should continue to do everything to treat her. After all, Marjorie did elect to have the surgery done so she could live. Marjorie did not have a Healthcare Power of Attorney.
