Respiratory Case Study Case Study: Med/Surg Perioperative Mrs. Jones, age 68 is being admitted to your surgical floor at 8:00 a.m. after having undergone surgery for acute cholecystitis. She had a laparoscopic cholecystectomy and has one peripheral intravenous (IV) line, and four small abdominal incisions with skin affix (a skin adhesive used to close wounds from surgical incisions). Her abdomen is soft, and her bowel sounds are hypoactive. Heart sounds are strong S1 and S2, her rate is 80 beats per minute, normal sinus rhythm. Initially her oxygen saturation is 96% on room air, lung sounds are clear bilaterally. Her IV is in her left forearm with a 20G cannula and LR infusing at 100mls/hour, her IV site is clean, dry, and intact. The PACU nurse tells you she has received 1000mls of lactated ringers (LR) and this is her second bag of fluid. The estimated blood loss from surgery is 5mls. The patient is stable, awake, and alert, she reports pain of 4 on the 1-10 pain scale. She was given a total of 1mg of Dilaudid IV in the PACU for her pain. She was given Zofran (Ondansetron) 4mg IV for prevention of nausea and vomiting. She denies nausea and has taken a few chips of ice.
