Original Question

CT is a 14 year old Hispanic female, visits you for a pregnancy test after missing her last 2 periods. CT is in a monogamous relationship with her 15 year old boyfriend and not using any consistent birth control. CT is not currently on any medications and has mild intermittent asthma (but no inhaler available). This is the first time CT has been to your clinic and to any health care provider in 6 years. CT is a freshman in high school. She participates in student government and has good grades. CT states her parents were never married and she has had no lifelong contact with her father. Her mother was 16 years old when she had her and she has no siblings. CT’s mother works several jobs to provide food and shelter. CT does have a large extended local family and they have lived with her grandmother off and on over the years. On testing, urine HCG is positive. CT is quite distressed to learn this result. After calming down, she is most concerned about telling both her mother and her boyfriend. CT is sure she will need to keep the baby as she sees no alternative. Based on the scenario above, please answer the following questions: What are some of the medical and psychosocial concerns posed by this adolescent pregnancy, and how do they affect the adolescent’s ability to cope with other life issues? How do these apply to CT? What kinds of resources could you, as an advanced practice nurse recommend to CT to help guide decision-making associated with health care and life choices? What are some methods that might be used to screen for an adolescent mother’s (CT’s) health and well-being? Are there any social determinants of adolescent pregnancy in general and applicable to CT? How do you screen adolescents for pregnancy risk? What are social bias encountered by adolescent mothers? What type of preventative program could be used to reduce adolescent pregnancy? Discuss second pregnancies in adolescent moms. Explore ethical concerns that you may encounter in your care of CT.
