HPI: The patient noted over the last 3 days significant swelling, erythema and marked worsening pain with drainage over the last 24 hours of his right great toe. He has had some infection of the toes in the past but no major infections. Problem 2: Type 2 diabetes. The patient has been on insulin for the last several years. He states he has lost 40 pounds in the last several months and is trying to bring his diabetes under better control. His BMI is 41. He does have numbness and pain in his lower extremities consistent with diabetic neuropathy. He denies any known kidney disease, eye disease or macrovascular disease as complications to date.
