She has hx of epilepsy unspecified, MDD, anxiety disorder, alcohol abuse with withdrawal and PTSD. He is currently on Buspirone 15mg BID for anxiety Disorder Bupropoion 75mg XR for MDD at HS levETIRAcetam Oral Tablet 500 MG (Levetiracetam) BID QUEtiapine Fumarate Oral Tablet 50 MG (Quetiapine Fumarate) FOR ALCOHOL rabuse and alcohol related psychosis. The patient’s quetiapine was decreased to 25mg on 5/24 and it resulted to increased psychotic symptoms, severe agitation and outburst. The patient notified the provider he is not able to function at a lower dose. Staff was consulted and the also noted the change in psychotic symptoms with the decreased in the quetiapine dose. The plan is to increase quetiapine back to 50mg daily.
