Endocardial ligation, left atrial appendage (prophylactic for risk of clot formation in this area) DESCRIPTION OF PROCEDURE: I performed a median sternotomy. The heart and aorta were exposed. I then applied caval snares. The right lateral aspect of the heart was approached. I divided the atrial septum. I entered the left atrium and extended the atriotomy toward the right pulmonary artery and beneath the IVC. The left atrial appendage was closed with running 4-0 Prolene suture in running fashion. I then inspected the mitral valve. The annulus was quite dilated. I therefore replaced the mitral valve. I sized the annulus and chose a #25 Hancock II porcine bioprosthesis, and 2-0 Ethibond pledget sutures were placed circumferentially around the mitral valve annulus. The valve was seated and secured.
