During this assessment, the patient received the worse news ever, her only brother and sibling alive that they are very closed passed away. This news was brought by her daughter that they share a difficult relationship. Her daughter was initially scared and fearful to come into the clients room. However when daughter told her, they both became tearful, hugged each other and promised each other forgiveness and to live right. She said her brother was the only reason she never remarried because he doubled up as a husband and a brother. She said she told herself two days ago that that she was better off death and she now regrets it because it felt because she said that, it made her brother to die. She was very tearful, sad, anxious during this assessment. Plan is to increase hersertraline to 100mg daily. Start hydroxyzine 25mg BID X 2 WEEKS.
