Melody is a medication-endorsed enrolled nurse in an aged care facility. On 26 January at 9 am, Melody heard a loud cry from one of the resident rooms. She found that one of the residents- Sam, had attacked Bill, another resident who was wheelchair-bound. Bill is holding onto his shoulder and crying out in pain. Sam is an Indigenous person and Sam said he attacked Bill because Bill made an inappropriate comment about his cultural background. Melody separated the residents with help from her colleagues and assessed Bill for injuries after gaining his consent. She noticed Bill had bruises on his right shoulder and collarbone. Bill cried out in pain while attempting to move his right hand. The registered nurse (RN) arrived and took over the assessment of the client. Melody applied some ice packs to Bill’s shoulder, applied a sling to immobilise the arm and administered pain relief to Bill. RN called the locum doctor and discussed the situation. It was advised to monitor Sam and to transfer Bill to the nearest hospital. Melody called an ambulance as advised by the RN to take Bill for further assessment and management at the nearest hospital. Once Bill was comfortable, Melody took direction from the RN and rang Sam’s and Bill’s families to inform them of the incident.
