HPI : I am here to establish care. I used to have a psychiatrist at Mulitcare but the last time I saw her was in December. I stopped going there because my therapist left and I felt like I was not really connecting with my previous psychiatrist. She said she was using Fluoxetine 20mg (TAKING 3Capsules daily)daily, Guanfacine 3mg daily and Adderall 25mg XR, and Vitamin D 5000unit daily. She said she has not been on medications since March. she said she had tried Methylphenidate in the past for ADHD abut it was ineffective. She said the Fluoxetine did not help with her mood and she will not like to continue taking it. She said she is currently feeling numb for the past week and she does now know why. She said sometimes she is just trying to enjoy her day and that she has this whole different feeling. She said she was in 12th grade but got withdrawn because she could finish her assignments. She said she had to even switch high school but still could not focus. She said no matter how she tried. She said she struggles with daily task like washing dishes, she said even when her dad will ask her to do the dishes she will keep procrastinating. She said she has been sleeping late and describes it as a love and hate relationship. Sometimes she sleeps too much and sometimes she is unable to sleep. Patient was educated on sleep hygiene.
