The patient claimed that the lump had been there for two years but only recently became sensitive to touch. On examination by a clinician, the mass was measured at 8mm and described as cystic in nature and mobile beneath the skin when palpated. She denied additional lumps and any recent trauma or illness. The skin in the area of and around the lump was unremarkable. Results from an ultrasound of the right side of the patient’s neck showed a slightly lobular 1.1 x 1.1 x 0.5 cm ovoid lesion within the subcutaneous fat. The lesion appeared to have two fatty hila with central vessels, prompting the radiologist to interpret the mass as a benign bilobed lymph node. The lump was surgically removed for further testing to rule out malignancy and identify the cause. During dissection, the lump was correctly identified as a benign sweat-gland tumor (eccrine spiradenoma). Since there was no infection, only pathology assays were done. The hematoxylin and eosin (H&E) stain is a commonly used tissue stain that dyes nuclei blue-purple and cytoplasm pink. It revealed a sharply circumscribed, multilobulated basaloid nodule composed of two cell populations, small, hyperchromatic cells at the periphery and larger, pale cells centrally clustered around eosinophilic basement-membrane globules.
