Respond to this discussion post: I would like to discuss the Transtheoretical Model and it’s importance to healthcare. This theory can be very beneficial because it has the ability to cater to all ethnicities and ages. I am a strong believer that for better patient outcomes you must get to understand where patients are during a certain timeframe in their life. We must learn to meet patients where they are and this model recognizes that change is a process and will not happen overnight. The (TTM) assist healthcare providers in forming interventions that are patient centered which creates a more individualized care plan that ultimately will increase a better patient outcome. The theory has 5 stages referred to as the Stages of Change including precontemplation, contemplation, preparation, action and maintenance. This theory encourages long term behavior changes as it’s last stage is maintenance. For the remainder of this course the topic from the Healthy People site I will be discussing is Diabetes. Diabetes is a known epidemic for the US. In the state of SC where I reside there are approximately 533, 300 adults who have been diagnosed with this disease (ADA 2025). There are very serious complications related to uncontrolled diabetes. Working in my current role as a post-surgical nurse manager I am constantly rolling out education on diabetes and assisting with patient education upon discharge. Working in my area it is important to have a clear understanding of untreated diabetes and how it relates to proper wound healing.
