Patient reported concern of not been able to see his fellow veterans’ friends since his hospitalization Patient reported he has history of suicidal ideation in the pasts but denied he do it. He is currently not on any psychiatric medication. Patient reported not eating well too. Patient reported remembering his encountered wars 70% of the time since his hospitalize. Patient’s daughter reported that patient was treated with Remeron and propranolol in the past for PTSD. PMH: Medical History: UTI Psychiatric History: PTSD as per daughter’s report Medications: Currently not on any psychiatric medication Allergies/Intolerance: N.K.A. Family History: No family history Social History: Denied history of alcohol and illicit drugs use. Married with 1 daughter. Retired veteran. Does not sleep well due to nightmares. Poor appetite.
