A patient has a transrectal ultrasound-guided placement of prostatic radiation palladium seeds with a cystoscopy for localized adenocarcinoma of the prostate. Procedure Description: The patient was given general anesthesia, placed in the lithotomy position, and prepped and draped in sterile fashion. The bladder was drained, and 100 cc of half contrast and half saline were placed into the bladder. The scrotum was then draped up out of the way. The BUK 7.5 MHz transrectal ultrasound probe was then introduced into the rectum, and the prostate was imaged. The probe was placed into the stabilization bar mechanism. We then centered the prostate image on the template screen and established our base image. Stabilization needles were then put into position. We then passed the catheter using a perineal approach at the corresponding positions to the corresponding depth, using ultrasound and fluoroscopy for guidance. After satisfactory placement of all the needles, the ultrasound probe and needles were removed. A total of 60 palladium seeds were put in place via transperineal catheter. We had good distribution of the seeds and good images on the ultrasound. Using the #22 French cystoscope, a cystoscopy was performed, and the bladder was fully inspected with the 30- and 70-degree lenses without notable findings. The bladder demonstrated no tumors, lesions, or other abnormalities, there were no seeds present, and there was very minimal bleeding. The bladder was drained and the patient was taken to the recovery room in stable condition. The procedure was performed in the cancer center.
