The service provider must be in a school or district classroom, office, or other appropriate setting (including home or hospital if services are provided there) when the interview is conducted. Interview questions must include the required questions for each Service Provider, plus 2-3 questions of your own. Interviews must be transcribed such that the full content of the interview is clear.
