When a patient sustains burns to large surface areas of their body, it affects the skin, nervous system, cardiovascular system, immune system, gastrointestinal system, and renal systems. Burns may be sustained from any sort of heat source as well as chemicals, and are classified as “partial-thickness,” or “full-thickness,” depending on the level of the dermis burned through (Å»wierełło et al., 2023). I will be referring to full-thickness burns due to the extensive damage they can cause to multiple organ systems. When a full-thickness burn is first sustained, it causes a hyperinflammatory response of the entire body as well as massive fluid loss due to vascular permeability. This fluid loss causes the heart to compensate for the decreased systemic vascular resistance by increasing blood pressure. There is also hypermetabolism where the body is trying to utilize all of its glucose resources to keep the vital organs alive, but at the cost of muscle tissue wasting and proteolysis, or the breaking down of protein. Burns are also noted to affect the gut microbiota as the body shifts from focusing on digestion to a hyperimmune response (Dobson et al., 2024). This means you will see a patient with extensive burns requiring significant fluid replacement, protection of the airway, and assistance with thermoregulation as a result of the dermis being so exposed (Miller, 2023). Significant burns will affect every activity of daily living in the way that it will take an extensive amount of time, surgeries, healing, and physical and emotional therapy to recover from a severe burn.
