I selected an article on Multiple Sclerosis (MS), which is a chronic inflammatory, autoimmune, demyelinating disease of the central nervous system (CNS). The article discusses pathophysiology, management, and the importance of primary care. Link: https://doi.org/10.1097/01.NPR.0000000000000083 MS is a chronic condition that contributes to demyelination in white matter, axonal loss, and gray matter damage. The myelin sheath is the covering around nerve fibers and is made up of oligodendrocytes in the CNS (McGuire et al., 2023). An exact cause of MS is not known, but the current understanding is that both environmental and genetic factors contribute to its development. There are more than 200 genetic variants are associated with susceptibility to MS and environmental factors associated with susceptibility include low vitamin D levels, obesity in early life (an association stronger in females), infection with Epstein-Barr virus, and cigarette smoking (McGuire et al., 2023). There are two hypotheses in the development of MS. The “outside-in” hypothesis posits that MS is caused by a peripheral autoimmune activation of T cells against CNS antigens resulting in immune-mediated demyelination.15 The “inside-out” hypothesis suggests that a primary event in the CNS triggers a secondary immune activation resulting in gray and white matter injury (McGuire et al., 2023). There are varied disease courses depending on inflammatory components or progressive demyelination. Diagnosis is made by patient history and MRI findings. Ruling out other neurologic conditions may also be indicated. The symptoms of MS greatly impact quality of life. These symptoms include fatigue, difficulties with memory or attention, depression and anxiety, bladder and bowel dysfunction, sexual dysfunction, pain or altered sensation, imbalance, muscle spasticity, weakness, and difficulty walking (McGuire et al., 2023). Maintaining good health is important to prevent the progression of MS. Regular checkups and screenings for other chronic conditions is needed. MS patients face a multitude of physical, emotional, and cognitive symptoms, which can have a direct impact on quality of life. Addressing social elements in a culturally aware manner is essential along with therapies to slow progression. 3 key points 1) Maintaining overall good health is an important factor in preventing the development of chronic and progressive diseases. 2) MS requires a multidisciplinary approach involving the patient, family, primary care provider, neurologist, social worker, and therapies.
