What medications are they currently taking? Nia is a 35-year-old patient that presented to the clinic complaining of reduced libido, weight gain, and insomnia for the past three months. She was started on 50mg Zoloft oral daily six months prior and her anxiety decreased but are still experiencing these side effects. The patient is requesting a new medication that will help with the anxiety but have decreased side effects. Her only medical history is generalized anxiety disorder, has an allergy to penicillin, does not smoke or drink, and Zoloft is the only medication she is taking. 2. Assess the applicable clinical practice guideline (CPG) for your assigned client. What treatment is recommended by the CPG for your client’s situation? Based on the CPG, SSRIs are the initial step for Generalized anxiety disorder. However, because of the adverse effects the patient experiencing the CPG recommends switching to a different SSRI. The CPG explains that escitalopram (Lexapro) is effective and well tolerated by patients. Starting at a lower dose of 10mg once daily of Lexapro is recommended as well as stopping the Zoloft when starting the Lexapro. Finally, a follow up should be done with the client in 6 weeks to ensure the medication is working and is tolerated by the patient. 3.
