Peripheral Vascular Disease (PVD) Pathophysiological Mechanisms of PVD Peripheral vascular disease (PVD) is a condition affecting blood flow due to obstruction in the body’s blood vessels. The vessel elasticity or diameter is reduced, resulting in blood flow obstruction. Many factors play into the pathophysiological development of PVC. Atherosclerosis begins with injury to endothelial cells that line arterial walls. These injured endothelial cells become inflamed and unable to make normal amounts of antithrombotic and vasodilating cytokines. Instead, they express adhesion molecules that bind to macrophages and other inflammatory and immune cells, further injuring the vessel wall. The inflammatory process associated with endothelial cells generates toxic free radicals through the oxidation of LDL. Oxidized low-density lipoprotein (LDL) recruits monocytes that differentiate into macrophages, ultimately engulfing the oxidized LDL and forming foam cells. A significant number of foam cells form a lesion called a fatty streak. This fatty streak produces more toxic free radicals, secretes additional inflammatory mediators, and causes further damage to the vessel wall. Macrophages release growth factors that stimulate the proliferation of smooth muscle cells. Smooth muscle cells near endothelial injury proliferate, produce collagen, and ultimately form an atherosclerotic plaque over the fatty streak. These plaques bulge into the vessel lumen and obstruct blood flow. Thus, atherosclerosis, endothelial dysfunction, and inflammation are all associated with PVD (Parwani et al., 2023; Rogers, 2022). Risk Factors for PVD There are many risk factors for PVD, including modifiable and non-modifiable ones. Modifiable risk factors are those that individuals can modify to influence the progression of a disease. Non-modifiable risk factors related to one’s propensity to develop a disease cannot be altered. Non-modifiable factors include age, sex (male or female after menopause), and a positive family history (Rogers, 2022). Modifiable risk factors include dyslipidemia, physical inactivity, hypertension, cigarette smoking, diabetes and insulin resistance, obesity, and diet (Barrios et al., 2022; Rogers, 2022). Epigenetic patterns contribute to the non-modifiable risk factor of age, while aging and menopause are associated with increased exposure to risk factors and poor endothelial healing. Low-density lipoprotein (LDL) plays a significant role in plaque formation. Hypertension causes endothelial injury in atherosclerosis and is associated with the overactivity of the renin-angiotensin-aldosterone system (RAAS), which also contributes to the development of atherosclerosis. Cigarette smoking damages endothelial cells, and nicotine stimulates the release of catecholamines, which increase heart rate and peripheral vascular resistance. Diabetes and insulin resistance impact the cardiovascular system and are often accompanied by dyslipidemia. Obesity is associated with inflammation, insulin resistance, lower HDL levels, and elevated blood pressure. Diet plays a significant role, as the amounts of saturated and unsaturated fats contribute to blood lipid levels, among other factors (Rogers, 2022). Manifestation of Clinical Symptoms of PVD PVD presents a variety of signs and symptoms, including intermittent claudication, rest pain, and critical limb ischemia. Intermittent claudication refers to leg pain that occurs during ambulation due to obstructed arterial blood flow in the iliofemoral vessels, resulting in ischemia. If the obstruction evolves into a thrombus over the atherosclerotic lesion, complete obstruction can lead to critical limb ischemia. Severe pain, loss of pulses, and changes in skin color in the affected extremities characterize this condition. Critical limb ischemia can eventually progress to gangrene. Rest pain may occur in the legs and feet affected by PVD, presenting as severe discomfort that disrupts sleep (Parwani et al., 2023; Rogers, 2022). PVD and Systemic Circulation Patients with peripheral vascular disease are at an increased risk for cardiovascular disease, myocardial infarction, and stroke. Peripheral vascular disease and cardiovascular disease both involve the process of atherosclerosis. Atherosclerosis affects vessels throughout the body and is the leading cause of peripheral vascular disease, coronary artery disease, and cerebrovascular disease. Thus, PVD increases one’s risk of myocardial infarction and stroke. PVD also indicates potential damage to the coronary arteries, while increasing stress on the heart due to its resulting decreased blood flow in the extremities (Rogers, 2022).
