Patient has history of MDD, GAD, Alzheimer disease, Dementia without behavioral disturbance. During last visit, Patient’s Desvenlafaxine 100mg for MDD was d/c due to concerns on effectiveness. Patient was then started on Setraline 50mg daily for MDD (both Patient and family consented to it. During her assessment today, she said nothing has really changed, she is till the same but she sleeps during the day a lot. She is currently on Trazodone 50mg at HS, Donopezil 5mg daily, Sertraline 50mg daily and Hydroxizine 25mg at HS. Patient said she dreams through out the night and it affects her sleep. She said she dreams and worries all night if her children are doing ok. She said regarding appetite, she has to force herself to eat not because she feels hungry.
