This is a 71-year-old gentleman with previous history of hypertension, smoking, chronic pain who is presenting to ED for evaluation of chest pain while at Madison Manor. Patient currently reports complete resolution of his chest pain. Patient has a nonconcerning story for ACS. Plan to obtain EKG troponin, basic labs, chest x-ray and reassess. EKG is nonischemic. Initial troponin is negative. Patient has remained chest pain-free. Second troponin negative, chest x-ray with no acute findings, patient remains asymptomatic. Patient advised to follow-up with his primary care doctor and cardiology for reevaluation. The patient was discharged from the Emergency Department in stable condition with written instructions regarding recommended treatment and follow up. A copy of these instructions is included in the patient’s electronic medical record. I discussed with the patient/patient’s family the importance of close follow up with primary care and specialist providers as applicable.
