Original Question

rephrase and shorten “Closed head injuries are specific, grossly observable skull and brain lesions that occur in a precise location. Of blunt trauma injuries, 75% to 90% are mild. Injury to the cranial vault, vessels, and supporting structures can produce more severe damage, including contusions and epidural, subdural, subarachnoid, and intracerebral hematomas. The dura mater remains intact, and brain tissues are not exposed to the environment. Compression of the skull at the point of impact produces contusions or brain bruising from blood leaking from an injured vessel (Fig. 18.2). The injury may be coup (injury at site of impact) or contrecoup (injury from the brain rebounding and hitting the opposite side of skull) or both (Fig. 18.3). The severity of contusion varies with the amount of energy transmitted by the skull to underlying brain tissue. The smaller the area of impact, the more severe the injury because of the concentration of force. Brain edema forms around and in damaged neural tissues, contributing to increasing intracranial pressure (ICP) (see Chapter 17). Multiple hemorrhages, edema, infarction, and necrosis can occur within the contused areas. The tissue has a pulpy quality. The maximal effects of these injuries peak 18 to 36 hours after severe head injury.
