The patient was admitted with C diff colitis and perforation Of gallbladder in septic shock, and underwent total colectomy w/ileostomy and cholecystectomy. Episodes of SVT occurred after surgery. Additionally she has had several episodes of obstruction while titrating medications due to high ileostomy output. So she received the revise procedure for stoma. Postoperatively, the patient continued to titrate high ileostomy output medications. But the patient was noted to have continuously tachycardia again and was diagnosed with acute PE w/o heart strain and transfer to ICU. After the conditions was stable, the patient currently received heparin and continuous Imodium. At the same time, dietary modifications w/calorie counts,increase fluid and electrolytes replacements, and strict I/O to address and improve high output stoma. The patient has the medical history with Sprue Celiac, Tourette’s Syndrome, Intellectual Disability and Malnutrition.
