make this sound better Action: During this rating period Mr. Ryan demonstrated the ability to use the nursing process when a facility nurse educator brought to Paul’s attention that the chart review process in the facility was lacking. This was identified during VISN 12 Clinical Reviews of Tomah Patients. Paul had been one of the individuals mentioned to the education nurse as a RN that does a through chart review. Paul then set up a time with the education nurse to walk her through the chart review process. The education nurse and Paul thought that the best way to fix the inadequate chart review in the facility would be to create a checklist to standardize the process CLC wide. Paul created and presented the completed checkoff sheet to the NPPC on 9/18/24 and is rolling out the checkoff list to all CLC units. This project is currently scheduled to be presented in the nursing grand rounds in March. Population: All veterans and staff within the CLCs Outcome: This is sustainable because all CLC units will now have a standardized checkoff list that will strengthen continuity of care. Paul was also asked to help create a competency for chart review as well as update the 24-hour chart review SOP for the facility. This sheet has already been a resource for Paul and other RNs he works with on 401A and has resulted in a more through chart review process that follows VA policy SOP-PCS-NUR-56.
