Robert is a 67-year-old male with a history of atrial fibrillation, gout, and type 2 diabetes. He was admitted to the hospital with a left MCA territory stroke and underwent emergent thrombectomy. Immediately after the thrombectomy, Robert experienced a period of neurologic recovery. However, he then experienced a su change in status. Robert became encephalopathic, leading to the need for intubation to protect his airway. Robert developed aspiration pneumonia and progressive renal failure, requiring dialysis. He underwent percutaneous tracheostomy. Over several weeks, Robert demonstrated recurrent fevers and persistent leukocytosis. He received multiple courses of empirical broad-spectrum antibiotics and serial cultures. He was evaluated for atypical pathogens, had cross-sectional imaging, lumbar puncture, and autoimmune serologies, which did not reveal a clear etiology. Robert was also trialed on bromocriptine for presumed central fever. On repeat physical exam, Robert was noted to have multiple swollen and tender joints. At this time, an arthrocentesis was performed and was positive for monosodium urate crystals. Robert was started on dexamethasone and later switched to anakinra due to uncontrolled hypoglycemia, with resolution of fevers and articular symptoms.
