Original Question

CPT code and modifier if needed for the following: Procedure: 1. Probing left nasolacrimal duct 2. Balloon dilation, left nasolacrimal duct Reason for Visit: Nasolacrimal duct obstruction, left eye PREOPERATIVE DIAGNOSIS: Nasolacrimal duct obstruction, left eye POSTOPERATIVE DIAGNOSIS: Nasolacrimal duct obstruction, left eye PROCEDURE PERFORMED: 1. Probing left nasolacrimal duct 2. Balloon dilation, left nasolacrimal duct DESCRIPTION OF PROCEDURE: The patient was taken to the operating room and given general anesthesia by mask. The left upper punctum was then dilated and a size-0 Bowman probe passed from the upper punctum into the lacrimal sac and down through the nasolacrimal duct with a small amount of resistance. A second probe was then passed under the left inferior turbinate and definite metal-on-metal contact established. The original probe was then withdrawn and a #2 LacriCATH was inserted from the upper punctum into the lacrimal sac and down through the nasolacrimal duct. Its position was confirmed in the nose. The balloon was inflated eight atmospheres for 90 seconds. It was then withdrawn 5 mm and again inflated with atmospheres for 60 seconds. At this point, the LacriCATH was withdrawn. A lacrimal cannula attached to a syringe containing normal saline dyed with fluorescein was then inserted from the upper punctum into the lacrimal sac and fluid freely irrigated into the nose and recovered with a suction catheter. A drop of a steroid antibiotic combination was then applied to the left eye, and the patient awakened and returned to the recovery room
