Reason for Encounter: Itchy rash History of Present illness: This patient is a 36-year-old overweight female with a PMH of type 2 diabetes who presents with tWO weeks of itchy, circular rashes on both thighs. The rash is red with raised borders and has been progressively worsening. The itching is constant and rated 6/10.She reports using an OTC topical corticosteroid for the past three days has made the lesions more red and caused a burning sensation. The patient denies fever, recent travel,exposure to known irritants, or any new skin products. She has no recent history of skin injuries, bites, or infections. Review of systems: Cardiovascular: Regular rate, rhythm. No murmurs, gallops, or rubs. Capillary refill < 3 seconds. Chest / Respiratory: Normal respiratory rate, effort, and excursion. Lungs clear to auscultation bilaterally. No use of accessory muscles. Skin: Multiple circular lesions noted on bilateral inner thighs, each with raised borders and central clearing, each approximately 1 to 3 inches in diameter. Associated erythema and scaling observed. No areas of fluctuance or palpable nodules. NO vesicles, pustules, discharge, or signs of infection.
