Also, some medical providers utilize online programs for their patients to enter their medical information to. The data developed in these source documents is important to ensure patient’s can easily complete them and medical care providers have a quick source to receive needed information in order to provide the best care possible to patients (Shanholtzer & Ensign, 2021, p.79). The source documents weather virtual documents or paper forms should include past medical history, review of systems, surgical history, family history, and social history. Social history includes tobacco, alcohol, and recreational drug usage (past or present). Also, socioeconomic data such as occupation, marital status, and education level is incorporated.
