Help with rewording please In terms of a care recipient receiving home care, we may be faced with a situation where the care recipient has chosen to relocate interstate to be closer to family. In this case, the location they are moving to may not be in a region where we currently operate or would be able to operate. This would meet legitimate criteria for the termination of the home care agreement; however, we would still assist the care recipient in sourcing a new provider, discharge our responsibilities for the transfer of unspent funds, continue providing care and services until they relocate, and provide a safe and effective handover of care including the provision of information to the new provider, and others such as the care recipients family, representative, or carers, in line with their consent. We could also experience situations where a care recipient exhibits responsive behaviours, for example, verbal or physical behaviours that could result in distress to staff, staff dissatisfaction, staff requesting exclusion from service delivery to that care recipient, or pose health and safety risks to staff, and which ultimately impact on the safety and quality of care delivery. This is a unique and inherent risk associated with home care as staff often work alone in the care recipient’s home, maybe remote from ongoing support from supervisors and colleagues, and other interventions from clinicians and specialist bodies are not on hand. This situation does not immediately meet legitimate conditions for cessation of the agreement and must be managed for the benefit of the care recipient and staff until and unless all solutions have been genuinely trialled and exhausted. We will be required to engage with the care recipient, their representatives, and support staff to attempt an improvement for all concerned, and we must be able to demonstrate that we have done so. Only when we are confident that we cannot continue to meet the care recipient’s needs can we work with the care recipient to accept that cessation of care and assist them in transitioning to another more suitable provider or type of care.
