Paramedics immobilise her on a back board and apply a cervical collar. They inserted an 18 gauge IV cannula in her right forearm and have commenced Hartman’s solution. They administer Methoxyflurane for pain and transport her to the emergency department of a major city hospital. On arrival in the emergency department, Mrs Rodriguez is awake and able to recall the fall. She complains of pain on the left side of her chest, abdomen and in her left thigh. Her GCS remains at 15. She is breathing spontaneously at a rate of 20 bpm, Spo2 94, Heart rate 90bpm, BP 110/60mmHg and temp 35.6 degrees Celsius. After examination of Mrs Rodriguez in the resuscitation bay it was found that she has suffered the following injuries, fractured ribs 8-10, Fractured left femur and a ruptured spleen. Mrs Rodriguez remains in resus awaiting theatre and you notice she starts to deteriorate. Her vital signs are as follows, her GCS remains at 15, PEARTL 2+mm, BP 98/60mmhg, Pulse 110bpm, RR 28, Sats 88% on 2L O2 via NP and temp is 36.0 degrees Celsius.
