She is also complaining of some low back pain × 5 days. She denies burning, itching, pain with urination. She denies fever and chills. She has also had some nausea and vomiting over the same period. She last vomited yesterday. She vomited twice, she states, and, this past Friday, 4 days ago, twice. She has not vomited today. She has had good fluid intake. She has had a slightly decreased appetite, she states. The patient states that she has had some UTIs this pregnancy × 2. Otherwise, she has had an uncomplicated pregnancy. The patient denies a history of hypertension, diabetes, and pre-eclampsia. She states she has had some irregular contractions, no real contractions today. She denies vaginal bleeding. She denies gush of fluid. She denies abdominal pain. She has had good fetal movement. She has not had headaches, visual changes, or abdominal pain. Physical examination: Blood pressure 119/66, temperature 36.5, pulse 86, respiratory rate 18. HEENT: Within normal limits. CHEST: Clear to auscultation bilaterally. HEART: Regular rate and rhythm without murmur, rub, or gallop. ABDOMEN: Gravida, nontender. Vertex. EXTREMITIES: The patient has 1+ pitting edema bilaterally to the knee.
