Simulation Mrs. Mary Black is a 77-year-old adult hospitalized for a fractured hip.  She had surgery 2 weeks ago to repair it and is now ready to be discharged home to stay with her elderly sister (Ms. Tina Smith, age 70) while she recovers. Before the hospitalization, Mrs. Black lived independently in her own home.  While in the hospital, the pipes in her home froze and burst, flooding her home and making it unlivable until extensive repairs could be made. Her home insurance will not cover the repairs, and she does not have the money to pay for them.  Mrs. Black has lost some weight since her surgery, and her dentures do not fit well, contributing to difficulty eating and further weight loss.   Mrs. Black is ambulating with a walker, requiring assistance getting up from a chair, but is relatively stable walking once she is up. She has required assistance with personal care since the surgery. There has also been some mild confusion since surgery. Medication: acetaminophen 500 mg orally every 6 hours as needed. Mrs. Black has arthritis in both hands. Setting: Surgery unit meeting room. This is a discharge planning meeting. The surgery RN will start the meeting on the unit, and the home care team members will attend the meeting, along with the client and her sister, to help plan the transition to home.
