Sam has early-onset dementia and has behavioural and psychological symptoms of dementia. As requested by Sam, a meeting was done with Sam, his next of kin and elders of his community to discuss trigger factors and appropriate management strategies. Based on the assessment and care plan review, the following strategies were advised. Observation: Physical aggression or precursor behaviours such as entering other resident rooms and verbal abuse noted. Nursing goal: Prevent behaviours of concern in the client. Prevent harm from behaviours of concern for the client and other residents. Preventative Strategies: – Provided guided activities to Sam throughout the day as advised by the activity therapist to keep Sam occupied – If Sam is seen wandering in the corridor, monitor to avoid entering other resident rooms. – Encourage inclusion — facilitate regular family visits and involve the Aboriginal Liaison officer to assist with community engagement. – Avoid triggers — TV programs with Aboriginal and Torres Strait Islander history, asking Sam about his childhood or his memories of living in a foster home, asking Sam regarding details of his family, calling him ‘Sir’.
