At approximately 6:30 this morning, Luna Smith, a 55-year-old female, was involved in a motor vehicle accident (MVA) on her way to work. Her car collided with a tree at high speed. Ambulance service was dispatched to the scene immediately. At the scene, Luna was wearing a seat belt in the driver’s seat. She was alert and reported no loss of consciousness; the airbag had activated during the accident. A 20 gauge IV access was gained on the left wrist, and fluid resuscitation commenced. Luna was extricated from the vehicle using a spine board and C-collar. The following observations were recorded: Blood pressure (BP) = 100/70 mmHg, Heart rate (HR) = 120 beats /per minute, Respiration rate (RR) = 26 breaths /per minute, Oxygen saturation SpO2 = 95 % on room air, Temperature = 36.7 °C, GCS = 15 (E -4 , V- 5 , M – 6), Pupils are reactive to light, and their size is 3 mm, Pain score is 10 out of 10, Pale, cool, and clammy skin with a three-second capillary refill in peripheral areas, Open femur fractures in the right leg, with obvious deformity and large blood loss. In ED at 7:45 am, Luna was bypassed triage, and her ATS category was 1, so she was sent directly to the resuscitation bay. Upon arrival at the emergency department, Luna was conscious, and her airway was clear. C-Spine collar remains in situ, and precautions were maintained during the transfer from the trolley to the bed. The following observations were recorded: Blood pressure (BP) = 80/48 mmHg, Heart rate (HR) = 136 beats /per minute in sinus tachycardia, Respiration rate (RR) = 34 breaths /per minute, Oxygen saturation SpO2 = 88 % on 15L O2 vis a non rebreather mask, Temperature = 35.5 °C, GCS = 14 (E -3 , V- 5 , M – 6), Pupils are reactive to light, and their size is 3 mm, Pain score is 8 out of 10, Capillary refill > 3 seconds The weak pedal pulses on the right leg were present, and normal pedal pulses showed on the left leg. Luna was observed to be pale, diaphoretic, and with cyanosis around the lips. It was recorded that Luna weighed 65 kg, was 167 cm tall, and had no history of allergies.
